VITAL RECORDS

 

Birth Certificates

The Probate Office maintains birth records for anyone born in Jackson County. In addition, we are able to provide birth records for persons born in any county in the State of Georgia, as long as those records were filed with the State Vital Records Office. The Jackson County Probate Court also files amended and delayed birth certificates as well as Paternity Acknowledgements and processes new birth certificates for individuals legitimized by marriage. Certified copies of birth records may be requested in person or in writing via mail. The cost of certified copies of birth certificates are $25.00 for the first copy, and $5.00 for each additional copy obtained on the same visit. We accept cash, check, and money order. If obtaining a copy of a birth certificate in person, payment can be made with a debit or credit card for a small processing fee, but we are unable to accept credit card payments for requests made by mail.

Birth certificates can only be released to the named individual or family members with the following degrees of relation to the individual, and proper identification is required:

    • Parent
    • Grandparent
    • Adult Sibling
    • Adult Child
    • Legal Spouse

Please print and complete the following form: BC Request and mail the same along with a self-addressed stamped envelope and a current copy of your driver’s license or other state issued identification. Written requests should be mailed to:

Death Certificates

The Probate Office maintains death certificates for individuals who lived or died in Jackson County from 1919 to the present. If you are looking for death certificates occurring before 1919, you can check with the State Vital Records office but there is no guarantee that the State office will have one on file. Certified copies of death records may be requested in person or in writing. The cost of certified copies of death certificates are $25.00 for the first copy and $5.00 for each additional copy obtained on the same visit. We accept cash, check, and money order. If obtaining a copy of a death certificate in person, payment can be made with a debit or credit card for a small processing fee, but we are unable to accept credit card payments for requests made by mail.

Requests in person:

Death certificates can only be released to the named individual or family members with the following degrees of relation to the individual and proper identification and/or documentation is required:

    • Parent, guardian or temporary guardian
    • Living legal spouse or next of kin or the legal representative
    • State or Federal Governmental agencies needing a death certificate for official purposes
    • Ex-spouses with a tangible interest and beneficiaries of wills or insurance policies
